Individual Membership Registration Form

Please enter your name exactly as it appears on your government-issued ID. This government-issued ID should contain your photo. If your ID is not in English, please kindly use an international travel passport as your ID. Please note: Blockchain Association Singapore (BAS) does not validate the ID data provided to us. It is your responsibility to ensure that you submit valid information from a valid form of identification.
This should be the same photo-ID you'll be using to identify yourself on the examination day. The ID should be in English.
One-Time Membership:
  • Valid for 1 year from the date of payment
  • No automatic renewal – membership expires after one year
Annual Membership:
  • Renews automatically every year
  • A renewal invoice will be emailed to you annually
  • You can cancel anytime before the next billing date

Declaration:

  1. I affirm that all statements made by me on this form are correct.
  2. I understand that any inaccurate or false information (or omission of material information) will render my application invalid and that, if enrolled into the Membership based on such information, my membership may be terminated by the Institute.
  3. I understand that should I be unable to submit the required supporting documents when requested to, this membership enrolment application will be considered invalid and will not be processed.
  4. The Institute may utilize the personal information and personal data collected via this form for processing of membership enrolment application through data sharing. This sharing of information will lead to matters relating to a registration being processed in a more efficient manner and the applicant is being provided with the most efficient level of service possible.
